Respite Services

The Respite Program provides assistance and support to families and caregivers who care for an individual with developmental disabilities in their home.  These services are offered to adults and children. Futures Unlimited hires staff to work in the families' home to provide periodic relief for the caregiver.

Home Based Support Services

The Home-based support services program tailors services to help adults with developmental disabilities to live at home. The Family Assistance Program makes monthly cash payments to families of children with developmental disabilities.

A service facilitator assists the adult participant in designing supports to enable them to stay in their home, learn skills and get a job.

Families interested in learning about and/or applying for either of these services should contact their Independent Service Coordinator Agency to complete the PUNS (Prioritization of Urgency of Needs for Services) form at 815-844-7708.
