The Entrance Criteria for the Employment Services Program are as follows: 1. Resident of Livingston County, Illinois, or a surrounding county. 2. At least 18 years old. Persons 16 or 17 years old may be accepted under certain special circumstances, such as through a cooperative agreement with the school system. 3. Indication of a disability or functional limitation which might impair vocational performance. In other words, something that decreases ability to hold down or get a job. 4. Willingness to participate in agreed upon programs and services. In other words, must be willing to make an effort to come in as scheduled, to work on goals that you agree to work on, to follow rules, etc. Futures Unlimited, Inc works with people with many different types of disabilities. In our Employment Services Program, we work with people with disabilities such as Mild Mental Retardation, Learning Disabilities, Mental Health Disabilities/Diagnoses including Substance-related disorders, Physical disabilities and many others. In order to receive services from our Program, we must have documentation or have a way to obtain documentation of your disability. There are many types of documentation, but usually it would include school records, physician records, psychological evaluations, records from counseling, etc. Feel free to contact us if you have any questions about eligibility, documenting your disability or about the Employment Services Program.

You can apply online by clicking on the link below, come to the office and fill out an application, or call and request that an application be sent to you.
